MHHCC OFFERS FREE SPORTS PHYSICALS

Minnie Hamilton Health Care Center has announced the schedule for physical exams for all students intending to participate in sports and cheerleading for the 2005-2006 school year.

Physicals are required by the West Virginia Secondary School Activities Commissions (WVSSAC), and will be held at the Calhoun Middle/High School-Based Health Center (SBHC) at Mt. Zion.

Physicals will be offered on Wednesday, July 13th from 8:00 am to 4:00 pm. A make-up date has been scheduled for Wednesday, July 20th from 8:00 am to 12:00 noon.

There will be two providers present to help maintain a smooth and efficient flow. Students unable to make these dates may schedule physicals at MHHCC any time during the summer.

As with all services provided by the SBHC, no family will be billed for the examinations. If a student is covered by private insurance, the WV CHIP Program or Medicaid, those insurances will be billed.

The family will not be responsible for any co-payments or deductibles.

We would like to encourage any parent who is interested in having their child seen on July 13th or July 20th to please call 354-9244 to make an appointment.

When calling, please inform the staff that you are wanting your child to be seen at the SBHC.

A WVSSAC consent and physical examination form must be completed and signed by a parent or guardian prior to the exam. Forms may be picked up in advance at MHHCC, Rite-Aid, Parson's One Stop, Speedy Mart or Holbert's Store.

MHHCC says they are looking forward to another exciting year of Calhoun County sports action, and wish all the athletes good luck in the 2005-2006 season.
